data from form to two different fields on template

I am looking to create a label that uses data from the Form in two different fields on the Template with two different outputs. ie enter "GRN" in the form and the template outputs "GRN" in one field and "GREEN" in the second field. Same with say "BLK" to "BLK" & "BLACK".

  If you are not connecting to a database already you could create a simple database that has two fields, one the abbreviation and other the full description. The data entry box could be linked to the Database Filter and the GRN and Green fields link to the appropriate column in the database
