Show form action only working in Bartender Designer and not Print Portal

Any ideas why Show form action is only working for me in Bartender Designer and not Print Portal? I have three forms, the second form displays fine but the third is ignored in the browser and only works in Designer

  I should add, if I allow display of error messages, it states there were no templates to render and no forms to show. However the form to show is labelled correctly and displays if I print in Designer.

  Ahh just sussed it. Label qty was defaulting to zero. Don’t know why though or how to fix to default to 1
