Error When Assigning Multiple Data Points to a QR Code

I am trying to add 3 data points to a QR Code: Subject ID, Sample ID, and Matrix. The subject ID and sample ID are alphanumeric, and for some reason when the QR code is scanned- the sample ID is generated twice, and the subject ID is not generated at all. When looking at the data sources, all points are correlated with the correct column to pull from of an imported excel sheet. In the Transforms tab, I have placed a carriage return after each point and am unsure if this is contributing to the error. Has anyone else encountered a similar problem? And does anyone know how to fix it? Thanks!

No I have not seen that before Have you tried adding the 3 fields on as text objects and checking that the data is being pulled through correctly? If so you could drag those text objects off to the side of the label and in the QR code make each substring and Object Value field and link each to the appropriate text item
