Connecting MySQL - Database field empty

When trying to set up a database connection to a MySQL server, I cannot hit next. This leads me to believe that the Database field is required but nothing shows in the dropdown and there is no way to manually enter in a database name. Is there something that I am missing? I have attached images of this below.

  I'm getting a similar problem with SQL Server 2019. Did you manage to find a solution?
