Grid Level Label

I need to create a grid level shipping label on a 4x6 label.....

 

I have created my spreadsheet that holds the data I need on my label

• Style#
• Description
• Color
• Size
• QTY

I need to show on the label everything that is inside the box.  The good thing is that I will only have one style#, description, and color per box...  But I will have Small's through XXL's on the label with different quantities of each size....

 

This is what I need my label to look like.....

 

Style#    Description     Color    Size    QTY

1234      Tee-shirt         Navy     S         10

1234      Tee-shirt         Navy     M         15

1234      Tee-shirt         Navy     L          20

1234      Tee-shirt         Navy     XL        5
